Skene duct cysts form if the duct is obstructed, usually because the gland is infected. They occur mainly in adults. Cysts may become infected, leading to recurrent urinary tract infections UTIs and abscesses. Rarely, Skene duct cysts if large enough obstruct the urethra. Some are larger and cause dyspareunia.

The first symptoms may be similar to those of UTIs eg, dysuria. Abscesses are painful, swollen, tender, and erythematous but usually do not cause fever.

Diagnosis of Skene duct cysts is usually clinical. Most symptomatic cysts and abscesses are palpable adjacent to the distal urethra; however, a diverticulum of the distal urethra may be clinically indistinguishable, requiring ultrasonography or cystoscopy for differentiation. Symptomatic cysts are excised or marsupialized. Abscesses are treated initially with oral broad-spectrum antibiotics eg, cephalexin mg every 6 hours for 7 to 10 days and are excised or marsupialized. Diagnose Skene duct cysts by physical examination and, if needed, by ultrasonography or cystoscopy.

Excise or marsupialize symptomatic cysts, and treat abscesses with broad-spectrum antibiotics and excision or marsupialization.

Paraurethral duct cysts are retention cysts that form secondary to inflammatory obstruction of the paraurethral Skene ducts in females. The cysts are lined by stratified squamous epithelium due to their origin from the urogenital sinus. Typically appear as round or oval masses that are located just laterally to the external urethral meatus and inferior to the pubic symphysis. On MR imaging, Skene duct cysts manifest as round or oval hyperintense lesions just lateral to the external urethral meatus. They secrete a fluid that helps lubricate the urethral opening and are surrounded with tissue that swells with blood during sexual arousal. The Skene's glands are located in the general area of the vulva , on the anterior wall of the vagina around the lower end of the urethra. The Skene's glands are homologous with the prostate gland in males, containing numerous microanatomical structures in common with the prostate gland, such as secretory cells. The two Skene's ducts lead from the Skene's glands to sites on the surface of the vulva, to the left and right of the urethral opening , from which they are structurally capable of secreting fluid. The origin and production site of female ejaculation has not been proven. It has been demonstrated that a large amount of fluid can be secreted from these glands when stimulated from inside the vagina.
